Provides case management efforts to employees throughout care journey. Roles &
Responsibilities:
Clinical Case Management Central touchpoint for all dedicated employee case management efforts throughout the care journey In conjunction with Corning Chief and Assoc Chief Medical Officer, establishes care plans and directs clinical engagement of external partners as needed to achieve best in class results Maintains a positive relationship with local/regional/national/international health care systems in order to better advocate for employees and their families Care Improvement Identify and mitigate local/regional clinical care shortfalls through partnership efforts and contractual agreements as required Works closely with Corning's Health Services team and health care providers to reimagine successful Executive Health programs across the country Develops and maintains Health Services policies and procedures Critical team member in all quality improvement efforts Employee/Family Support Assesses and identifies opportunities to support the employee and their family members along their healthcare journey Educates employees regarding resources and services available Provides quality employee engagement and rapport building Furnishes health information based on health literacy level and specific area of interest Consumer Medical Liaison Serves as the primary liaison to Consumer Medical services in order to facilitate employee and family engagement to this critical vendor Partners with the extended Consumer Medical healthcare team to ensure outstanding clinical outcomes emphasizing the preferred provider network and second opinion services Required Skills and
Experience:
Family nurse practitioner with Master of Science degree in nursing Minimum 8 years corporate executive health experience Experience in a global corporate setting providing clinical navigation to employees and their families from diagnosis through treatment Experience communicating medical information to a non-medical audience Proficient computer and internet navigation skills Dedicated to providing high quality, professional, customer service Experience working with a variety of digital/internet media and interest in exploring new information technology Excellent written and verbal communication skills Enthusiastic, must enjoy working independently and in a team environment Must be comfortable with a fast-paced environment Must be able to multitask while providing excellent customer service to our participants Familiar and comfortable working at all levels of corporate medicine This position does not support immigration sponsorship.
Salary Range:
$100K -- $150K
Minimum Qualification
Nursing, Healthcare ManagementEstimated Salary: $20 to $28 per hour based on qualifications.
